Commonwealth Home Support Program (CHSP) for CALD communities
Funded by the Australian Government Department of Social Services, under the Commonwealth Home Support Program, CCCi’s CHSP (formerly HACC) Social Support Services is available for eligible residents in the Aged Care Planning Regions of Inner West and South East Sydney
Under this program, we assist service users to lead active lives, improving their well being and quality of life, and therefore better able to remain in their homes and in their communities living independently, minimizing premature admission to residential care institutions.
CCCi’s programs are designed to respond to the needs of service users with culturally and linguistically diverse backgrounds (CALD).
Our programs aim to provide service users with comprehensive support through social inclusion activities, promoting an active lifestyle and reducing social isolation, while at the same time providing respite for their carers.
What does CCCi provide under CHSP Social Support Services?
- Social Support Group (formerly known as Centre-Based Day Care) – social support and activities in a centre-based setting
- Social Outings – social activities outside the usual centre-based setting
- Social Support – Individual – including telephone and web-based monitoring, visiting, accompanying to social/medical appointments, or just providing company for a chat
- Transport Service– helping people get to a Centre, shopping or appointments
Our services are provided by bilingual support workers and/or trained volunteers who undergone a Criminal Record Check.
